The forms below are to request a transfer of records from your previous dental office. One form can be used for everyone in the family if they went to the same dental office, by adding each name and date of birth, to the request.
If any family member has had treatment in an office other than a general dentist, such as an orthodontist, oral surgeon or periodontal office, please use a separate form for each specialty office. Any of these offices may have current x-rays, which would be helpful to add to your record with our office.
